PAYMENT OPTIONS
Services are provided with a minimum of three (3) hours per visit. Rates will vary by the individual situation.
We accept the following payment options:
•Insurance
•Medicaid
•Private Pay (Checks, money orders, cash)

Insurance companies and/or Worker’s Compensation carriers can be invoiced directly or indirectly depending on your individual needs. We will verify insurance coverage so that you receive the maximum benefit possible. We will also assist you with the completion of required forms for insurance reimbursement. Call us to see if your insurance covers our services
· Private Party fees are established in advance and billed weekly for services rendered.
· As an added convenience, we accept private insurance and Medicaid.
